  1. Date of activity.
  2. Client name.
  3. Client address or unique chart number.
  4. Patient name.
  5. Amount used or dispensed.
  6. Balance after use.
  7. The drug handler’s initials (not just the doctor who prescribed or ordered the drug)

How must a controlled medication be logged so that a veterinary practice can keep track of it?

It must include the name, address & DEA registration number of the veterinarian, the date and time the inventory is taken and the signature of the person conducting the inventory. The written inventory must be maintained for two years.

Be kept for a minimum of two years after the date of the last entry. The BSAVA advise that for ambulatory vets carrying CDs in their vehicle, ideally there should be a separate register.
